If you are a new patient or have not visited our office in more than 3 years, please complete the new patient paperwork and bring the completed forms to your appointment. Check-in time is 30 minutes prior to your scheduled appointment time.
Patients can also fill out the new patient paperwork online by accessing our Patient Portal. Once registered patients can login to the portal to update medical information including medications and allergies anytime. Test results and office notes are available to view on the portal after signed by Dr. Doubrava.
To begin using the Patient Portal, contact our office to be given access and a temporary password.
*If you prefer to fill out the paperwork in the office, please arrive 30 minutes prior to your scheduled appointment time.
*We ask patients that you take the time to complete all necessary forms prior to your appointment. Our office strives to run on time to minimize wait times for all patients. Patients arriving 15 minutes late of their check-in time may be asked to reschedule. We require 24 hours’ notice if you are unable to keep your appointment.
What to Bring
Please bring the following items to your appointment:
- Completed New Patient Packet
- Allergy list and medication list including the dose and frequency
- Films and/or lab results related to your condition (if available)
