Dr. Russell Doubrava accepts most insurance plans, if you do not see your insurance listed below please call our office and we will be happy to verify your coverage. Please remember if your insurance plan (ex. HMO, POS, Managed Choice, etc.) requires a referral for a specialist visit you will need to contact your primary care provider to obtain the referral prior to making an appointment.
Our office will file insurance claims for each billable service we provide. Patients are responsible for deductibles, co-payments, and co-insurance as stipulated by your plan. Payment is due upon check-in or we reserve the right to reschedule your visit. We accept Cash, Check, Visa and Mastercard.
Appointments
If you are needing to schedule an appointment with Dr. Doubrava, please contact our office and our staff will be happy to make you an appointment. As a courtesy to our patients we do call the day before to remind them of their appointment.
If you are needing to cancel or reschedule your appointment, we require at least 24 hours notice if you are unable to keep your appointment. Failure to do so, may result in a $45.00 charge.
Prescription Refill Requests
Patients may request medication refills by calling our office or through your pharmacy. Patients requesting refills on medications please allow 48 to 72 hours for refills to be processed as Dr. Doubrava is not in the office every day of the week.
Dr. Doubrava requires an annual visit to (maintain/approve/authorize..) medication refills.
Nurse Visits
Effective October 1, 2016, our policy for Nurse Visits has changed to the following:
- Patients will need to schedule an appointment with Dr. Doubrava.
- For same day appointments, patients must call the office first (940-627-0088) and verify a time. Our office will try to accommodate you the best we can.
- If you feel that you might need a prescription urgently, you may want to see your Family Physician or Urgent Care.
